Waiver, Release, Acknowledgement

• I understand that inherent to the game of ice hockey, as well as other aspects of the game sessions, is the risk of personal injury. As the parent or guardian of this child, I’m fully aware of such risks and have explained them to my child so that he/she has actual knowledge of such risks.

• I declare my child physically fit to participate in the school and has no medical condition that restricts his/her participation.

• In the unfortunate event of an accident I agree not to bring or peruse legal action against Wizard Hockey, the city, the city’s ice arena, its owners, directors, employees or contractees as a result of any injury that may befall my child.

• I declare that I have adequate insurance coverage for any injury or accident that may occur at the school, and I grant the school my permission to seek emergency medical assistance for my child.

• I agree to hold the same parties as above harmless for any and all lost or stolen articles of property.

• I grant Wizard Hockey the right to use any photos or video footage of my child for instructional or promotional activities related to Wizard Hockey.

• I understand and have explained to my child, that they must behave in a responsible and respectful manner while attending the tournament or showcase, and that my child may be expelled from the school, without a refund of the registration fee, for failing to abide by the ice arena, tournament or showcase rules.

• I understand that this release is binding upon my heirs, assigns and executions.

Cancellation Policy

• In any and all cases of a participant’s cancellation for a 3-on-3 tournament, $50 of the registration fee is nonrefundable but will be applied towards any future Wizard Hockey event.

• In any and all cases of a team cancellation for a tournament, $300 of the registration fee is nonrefundable.

• No refunds are given for teams who cancel within 30 days or less of the tournament start date.

• I understand that I am not allowed to use any video equipment to record any Wizard Hockey events.

• No refunds are given for participants who cancel within two (2) weeks of the start of their session provided that a refund (less $50) will be given for extreme cases of hardship such as injury or illness as long as a doctor’s note is submitted to us.

• No refund is given to participants who Wizard Hockey by their own accord including expulsion for violation of arena rules or our event(s) rules.

• Any cancellation on our part will result in a full refund of the application fee plus a $25 voucher towards a future Wizard Hockey event.

Hotel Policy

• ALL TEAMS/INDIVIDUALS NEED TO BOOK THROUGH THE HOTEL PROGRAM OR YOU WILL RECEIVE A PENALTY FOR NON-COMPLIANCE. PLEASE SEE THE PROGRAM OUTLINE IN THE HOTEL SECTION.
